Presenting to the senior doctor and defending the case against objections is its own block with its own criteria. Here it is practised as one.

How it runs

Three blocks. Exactly like the exam day.

The simulation runs in one sitting. The timer runs on the server, you get a warning 60 seconds before the end, then the block moves on. One pause of up to five minutes per block is possible — the real exam has none, and the interface tells you so.

An AI patient with a personality and everyday speech. Some facts come out unprompted, others only if you ask, and one only appears if you probe with empathy. Arzt-Arzt-Gespräch

You present the case. The senior doctor speaks in a formal register, interrupts if your presentation runs past three minutes, and always raises at least two objections — for instance: „Warum schließen Sie eine Lungenembolie aus?“

What is assessed

  • Structure of the case presentation (40%)
  • Answers to the objections (40%)
  • Terminological precision (20%)

If the connection drops you get a 60-second grace period with automatic reconnection, and the state of the conversation is kept. If it does not come back, the block is assessed on what exists and is clearly flagged as incomplete in the report.

The result

The report tells you what to correct.

No overall grade, no percentage, no promise about your odds. Six dimensions with a label, the critical omissions by name, and at most three concrete reasons this could go wrong. Every statement is backed by a quote from your recording — anything that cannot be evidenced is not shown.

Each terminology finding in your report turns into a short drill. You say or type the sentence again, correctly, and the card closes. If the same mistake shows up in a later attempt, the card comes back.

Frequently asked

Is FSPreif official exam preparation?
No. FSPreif is a private training product. It is not affiliated with any Ärztekammer, not endorsed by one, and not part of the official procedure in any way. It is language training for the Fachsprachprüfung — not clinical training and not medical advice.
Which language does the simulation run in?
Always German — patient, Arztbrief and senior doctor. Only the interface and the report labels are available in German, English or Arabic. Quotes, technical terms and the report's section names stay German.
What happens to my recordings?
You are asked for consent before the first voice block. The transcript is stored because the report draws its evidence from it. The audio file is deleted automatically after 30 days by default; you may choose to keep it longer. Export and full account deletion are available to you directly in settings.
What do I need technically?
A current browser, a microphone and a stable connection. The conversation happens in real time, and you can interrupt the patient or the senior doctor just as in a real conversation.
What if the connection drops mid-conversation?
It reconnects automatically for 60 seconds and the conversation state is kept. If it does not come back, the block is assessed on what exists and flagged as incomplete in the report. An abandoned session can also be resumed at the block boundary for 24 hours.
Can I repeat just one block?
Yes. From the report you can restart, say, only the Arztbrief. Only that block is re-assessed, the report is recalculated, and it shows which parts were repeats.
What does the free attempt include?
One complete simulation on the demo case with the complete report — all three blocks, all six dimensions, omissions and risks. History and drill cards are not included.
Do I get a score or a probability of passing?
No, deliberately. There is no overall percentage and no statement about likelihood. The report gives a label per dimension and names concretely what to correct.
